I appreciate this development, Tetsuma seemed way too strong to be only one level higher than Kai/Inari/Sakura/Shouya. I would have expected the next level to look more like Oshyun than Tetsuma. It makes sense that BJ expected her to lose now. Ironically he didn't actually see her get oneshot...

It does make a bit of sense - remember that when Kyara is recalling his first time meeting Baku, he says Baku has nothing to offer but his body. He may have simply done the same and lost an earlier gamble, and got saddled with the bad heart.
