I think her sister last chapter was comparing her to how she looked on page 4 of chapter 1. She's definitely bigger than in that flashback, but I think the "fat" comment was an exaggeration. ...or it was a snide remark about her breasts growing larger (which I think is common during pregnancy).

She's definitely thicker in most places compared to before, like on chapter 1 page 4, but I wouldn't say she's fat (far from it, actually).
