I think page 22 is somewhat wrong storywise. Yes, Guts presence could provoke Casca to relive her traumatic experience but Miura presents him as some kind of pure evil in cahoots with apostles.
I hope he won't go for "I blame you for what happened" kind of story.

No. Karu inject poison and girls die normally. In chapter 14 guardians burned the body of a girl who was attacked by Karu.
On the other hand few Karu dissipated after critical hits one chapter before. And that somewhat resembles a moonbreaking.
You "predicted" this twist because Lynn planted some accents in the story. And I think this is much better than introducing some cliched aliens, A.I., mad scientists, etc. Hard to imagine how he could link everything (witches, time travel, Nishina, world state, etc) together beautifully.
Probably survived girl is Rino (cheery blonde). One can see her shoes and stocking. Makana (helmsman) is in question.
BTW. Regarding language discussion. They use japanese in everyday life in this world (see ch. 3).
Actually putting aside fanservice this is how real isekai should look like. Pretty tired of escapist fairy tales with invulnerable OP characters. Most of inhabitants of another world wouldn't be interested in your matters and some will try to use you. This remind me of Re:Zero a little.

No. Based on flashback she is bossy, pretty serious about art and was really encouraging for Paisen. Actually she is good fit for him. To prop his talent at least. No wonder he fell for her.
