Add in a report button for reporting bad uploaders.

recently my rss have been getting spammed by a couple of bad uploaders. There's no real way to report this bad activity and they just keep doing it.

There should be at the very least some type of IP-ban for uploading chapters and a report button for regular users like myself to report bad uploaders.

How do you "report a chapter"? There's no obvious way that I can see. Only a single report button for reporting the entire title as a whole.

@fasontales In the reader, on the right, there's a button with a flag on it.
