Whenever Dr. Du speaks I always get the impression of the pot calling the kettle black and transferring his insecurities. He's never met the MC before yet can denounce him as a fake at a glance? If he's so good why didn't he point out Xiang's condition and treatment as well? Considering how much is being paid for the uncle offering a helping hand for Xiang too for a discount or free shouldn't be asking much. Then again, Grandma Cheng seems to have some very good insight into the characters of both Du and MC. Since MC's plot armor is going to prove him right I have to wonder if Du is incompetent or prescribing something that'll hide the symptoms having been paid by the poisoner not to interfere. Wonder if it's Xiang? Lack of sleep and other symptoms could be signs of a very guilty conscience not to mention he's the one who brought Du over.
