Wasn't there really anything for the MC to use as a weapon inside that place? Any piece of metal or other hard material would have made all the difference when attacking from behind a big man like that false nurse. If it's a blunt object, hit the back of the head. If it's a sharp object, thrust below the rib cage, off the spine. Or in other words, aim for the kidneys.
