The doctor should add extra stuff to the surfer dude's bill, to compensate for having to suffer his presence. The tiny clinic needs more equipment, so it would go towards that. I doubt he can even take simple 2D x-ray images there.
This actually isn't as bullshit as it sounds, believe or it not. Especially in colder waters (and "colder" here means 71 degrees or less, and yes, you can still easily get hypothermia in waters that are even considered warm if you're in them long enough). Lower temperatures mean the brain uses less oxygen, which means you have a larger time frame where you can have drowned but still remain in a state where you can be resuscitated.
