I'm a fan of this manga and I recognize some licenses have been taken in order to make an enjoyable narration. However, no matter if Dr. Koto is the bestest pro MLG no scope 1080 surgeon, this arc felt way out of the pace and context of the story until now.

Yeah, I'm not a medic, so I don't have an objective point of view about the events described, but come on, a brain surgery in an island hospital, understaffed and almost without equipment for a possible eventuality? It sounds not only fictional, but negligible and almost criminal.

Well, successful trepanations were performed even in Ancient Greece, and they have MRI machine and such in the clinic ; and the operation itself involved only the removal of a tumor. I don't think it's the most egregious operation shown so far.
