Follows disappearing

Some of my followed titles that I could have sworn that I followed, occasionally pop up as unfollowed titles. did it get renamed or renumbered in a way that made it disappear?

Shouldn't happen. The only thing that comes to mind (besides you having made some kind of a mistake, I guess) is that sometimes people create duplicate manga entries, but usually they're dealt with by merging the entries which should preserve the follows so that shouldn't be the issue.
