Why is there Napoleon in the background?? Is this a fictional world or based on actual history?
. Is this not alternate reality, or the artist was being too lazy to create an original piece.

It's a common "people will use love to use you or use the people you love as your weakness" trope along the lines of "people will say they love you, to gain your trust, but it's fake and they will betray you", which is usually presented in some backstory.
Ultimately, it's just an easy excuse to allow "exposure effects" to develop into romantic feelings, under the excuse of "this person is close but it's ok, because there's no feelings, but oh why can't I stop thinking about them?!" ¯\_(ツ)_/¯
