I'm not sure how old the doctor is, but given he's gone through college, med school, internship, residency, and then working there for a while after before the divorce happened, he must be at least twice Sunya's age. She said she's "almost 16" (so 15 and change). Given she's super naive and sheltered and she hadn't even had her first period before, she seems to be a late bloomer, so it's even weirder. I think there'd have to be a serious time skip to make it not creepy.
