In a much more serious/edgy setting, "If he was awake I'd kill myself right now..." would be considered a red flag that she might not be all that emotionally balanced or sane.

The term "red flag" doesn't come from visual novels. It's a common expression in the U.S. to signal danger or in the case of relationships indicates the person would not make a good partner .
Maybe because they use red flags on beaches to warn people the water is dangerous. Or maybe the term is older than that. but it's been around well before VNs.
