Then when Naofumi escaped Raphtalia transformed back.

@Raydnt You're close but sadly wrong, Raphtalia didn't transform back it's just all in L'arc's imagination.

As you could see a couple of chapters ago, Naofumi and Rishia reverted back to level 1 that also means that Raphtalia also reverted back. Raphtalia is still 10 years old so when she turned back to level 1 her appearance also changed back to her original form.
