I, for one, give Index III a 3/10. I would much rather be left hanging at the end of Index II than have a subpar adaptation of the follow up.
I mean, before we got Index III, we still had the chance of getting Railgun S levels of quality for the third season. Instead, we got the third season that everybody agrees is just terrible.
I wish it didn't exist.

To my knowledge we still don't know who Mugino killed in this instance, doesn't seem to be Yonbou and the implication is that he/she died shortly after the school encounter. Would have to have been some encounter she ran into while attempting to apprehend school, some skill out remnant or anti-skill officer perhaps?
