Trying to find a certain manga to reread. help?

So some time ago i remember reading some manga about a college girl that had a heart transplant or something after an accident. and like, she started seeing memories of that person, and later meets the donor's remaining living family and after stuff happens they live together. i wanted to reread the manga because i really liked it but i can't remember the name of it for the life of me. can anyone people give me a hand with this?

Not sure if you have found it but which year was it (based on art style) or what genres it belongs too? When I searched in B-MU they only have limited manga limited about organ transplant
