Who is getting delusional? Shino because she mentions her brother when asked about family? You have to consider that the people at work are more or less the first strangers she really interacts with and she might not be comfortable with just dropping the line "Oh yeah my brother is dead". Honestly, I think Shino is/will be fine from what I can see depicted in the story. She mostly is over the shock and is starting to process her grief. Nozomi on the other hand...
Although your comment also made me look up the beginning of the manga and it says that at least 6 months have passed, so clinically speaking, this can be categorized as complicated grief. Talking with a counselor should be an option and possibly where Nozomi's arc leads her

I can really relate to Shino and her problems. My older brother died last year, but it's not something I feel comfortable discussing with strangers. When asked if anybody in the family is married, do I say yes, or no? Since my older brother was the only one who was married in my family. When asked how many siblings I have, do I reduce the number by one? If I said so, it feels like I'm fine with a family member leaving , which I'm not. I'm over the grief period, but there's still lines and boundaries to what I can think and say at this point. Even thinking about the answers leave me twitchy.

I think in the story it's completely fine and doesn't make her come off as delusional, as someone else mentioned. It's part of her grief and getting over it. Furthermore, if you consider that her brother was apparently all the family she had left (well, she kind of has Nozomi now), it's another reason for her to act like that. Having to ask yourself question like those that Dirtbug mentioned is clearly the road Shino is heading towards and that's good.
