From my interpretation, The "Form" that we see is actually just the clay doll from ch 26. I think ch 26 is saying that originally in the timeline Touka is hit by Truck-kun and is replaced by the Clay doll. Same thing happens to Hanada when She timeslips from 2000 to 2010, but this time she dies and gets replaced instead.

I honestly feel like no matter what the author did not everyone would be satisfied with the end, because he raised the expectation too high because of the story elements + plot + etc.
