The textual support of his sudden change is basically non-existent. There is minor and persistent textual support for an escalation of his violent tendences but there is no convincing trigger event for him suddenly descending into psychopathy. Being injured and Maria being mean to him have happened numerous times. The scene in the hospital was him expressing his helplessness and vulnerability to one of his only friends literally nothing about it implied that it was him deciding it was "Time for a bit of the old ultra-violence"
