I don't understand the the last lage page where the brother (top) said that he's the only one for his other brother(bottom) in relation from its previous page.
What does it mean? Is he already twisted?

From my understanding, I think they both like each other but they just don't know how or if they should express it. Real brothers don't really follow each other around as they do in this story. So....I believe they are more afraid of being separated from each other and that's why they don't express their feelings honestly.
