At first I thought the translator was being edgy for the sake of it (like using normalfag instead of normie for riajuu), but then I checked the raws and Doppo does call him ano okama. You can check for yourself here: https://yuki.la/a/175534634#p175537974. The translator even commented:

No, I'm not being colourful; that's what he says.

Though okama is not an easy word to translate in English: https://legendsoflocalization.com/okama-in-game-translation/. Besides homosexual it can also mean cross-dresser or effeminate man, so you may render it as something like pansy or cupcake to make fun of his looking effeminate (I know they sound silly and old-fashioned), or to be really edgy, that word that starts with T and rhymes with granny...
