Help with a manga title

Bugging the crap out of me that I can't remember the title of this manga I read a few years ago. MC kept writing journals about his ideas of an ideal girl, then when he finally makes up his mind to get rid of them and (I think) confess to his crush, something happens to somehow bring the dream girl to life and hilarity ensues.
