The joke about the name of the goldfish is that while "niisan" means older brother and can be used as an honorific after one's name - like "Tsuda-niisan" or something - "onani" means mast*rb*tion. So while the phrase "ona-niisan" could be interpreted as "big brother Ona", it could also mean "Mr. Mast*rb*tor."
