how are RAWs made

Do people just shove the book into the scanner, or do they take the book apart.
 
The lower quality scans are made without breaking the book, so the spine crease is usually visible.
The highly quality scans do indeed RIP and TEAR.
 
Buying physical raws used to be a big thing like I remember seeing so many cleaning tutorials over at DA here's one I bookmarked from almost a decade ago, and yes they do rip from the spine basically taking it apart then scanning them. I vaguely remember a group posting a picture of a stack of manga they destroyed and that it was such a shame. Nowadays most groups use digital rips.
 
I also think they just ripped the book and then scan them page by page, at least for older manga that doesn't have digital copy back then.
 
I want to know why some people write raw in all caps. Isn't it just the word "raw" as in unprocessed?
