I usually have the smut/ecchi/whatever tags disabled on Mangadex, because I don't come here for porn.
However I'm a big fan of weird mindfucks that have sexual themes. Stuff from authors like Shintaro Kago for example.
Wouldn't it be nice to have a search function to find weird NSFW works that aren't just fapping fuel?

Use the "psychological" in conjunction with the sexual tags, this should help filter a lot of it.
Otherwise you might want to look at the tags in mangaupdate and combine them as well.
Keep in mind that 99% of tag proposed here are rejected for a multitude of reason and this isn't likely to change soon.
