So, basically, the grand duke is an illegitimate child of the Emperor, who has a disease which makes his hand stony, and he needs medicine every 6 hours from the Emperor to relieve the symptoms. When he met FL, he realises she relieves symptoms and later will able to cure him entirely, therefore freeing him from the Emperor's control.
