@Maala not only blackmail, she's sort of got a hostage situation going, if he breaks Ruka's heart it will be literal, since she's made it clear she has a heart condition, where it barely beats to begin with. She's forcing him to date her, so that it will accelerate her heart rate, letting her feel alive.
