They are either childhood friends or cousins by the look of it. Even though the boy probably loves her.

Stretch has no undertones regarding them being anything else than a lesbian couple. In this manga it doesn't look like them being a couple like at all. She feels comfortable around him even while being naked or doing everyday tasks, so I think they are a lot closer to being family than them being lovers. Even though the boy is taken aback, when she told him about the "just like siblings" or something along the lines.

There's plenty of little hints suggesting romantic attraction in Stretch, some of them explicit but unrealized on part of their holders, others subtle in "romantic two
