It's kind of a running gag at this point. I haven't read the light novels the manga is based off of so in print the gender ambiguity may take more relevance or be non existent until there needs to be a reveal which with Japanese language is more likely. In the first case it would imply that Mikado is super dumb. In the second case Mikado is still super dumb but he is being poked fun of by the mangaka in relation to the original text which is a fun kind of clever.
