You're being kind of nitpicky, but regardless, the Middle Ages you're thinking of is for medieval European history, not Asian. It's set in the imperial court according to the manga description, which covers a time period of around 200 BC-1900 AD. Cacao was also discovered by Spaniards during the 16th century and became widely circulated during the mid-17th century.
 
This girl really remind me of Poison King Mithridates, i got a feeling that the author based her entire character of King Mithridates of Pontus

@wrex japanese classification is based on the magazine it's published in, it's not a tag relative to the content (though obviously mags try to get work which fit their target audience).

Kusuriya no Hitorigoto is published in a seinen mag' (Square Enix's Big Gangan) making it a seinen. That's all there is to it.

It's actually a double seinen since the LN has two different adaptations, both in seinen mags (the second is in Shogakukan's Sunday Gene-X).
 
Just binged it up to chapter 24, it's pretty good. Would definitely love to see an anime of this.
 
It's so rare to see a Japanese Historical story ( Don't know what you'd call this genre) I do love this, MC is best MC and big boob girls are the best!
 
Rare to see a good historical Chinese mystery crime solving political romance that's actually good. Love it even more so for the MC using her background knowledge in pharmacy and her wit to maneuver around the high court. The harem was definitely a battle ground back then lol. The Japanese author did their research, but I can't pinpoint the dynasty the story is set in.
