Everything killed outside the dungeons come out bullets other than the gorilla thing that became a gun.
But each monster on each dungeon floor becomes a specific item. If the cart is full of multiple items, then multiple monsters should be coming out of it. In the novel, only things that are broken or smashed up together become 'new' monsters. Like if you had a watermelon and a pumpkin smashed near each other they could both fuse into some new monster.
