No those on the photograph are probably still "harmless", relatively speaking. I mean look at the fucking eldritch abominations MC sees.

As to why? We're just left to guess at this point.
Maybe unless you're correctly "attuned" or something their presence is so overwhelming that you can't grasp their form properly? The old lady could see the shadow of a stronger one and recognized it as such, so it's clearly on some kind of scale, not an either or thing.
