Why not both? Since you are translating, being "Mr.xxxx-san" would make sense, cuz you get the contents from one language to another

I prefer using "-san". it makes it easier to understand when different suffixes are being used (e.g. dono, or sama) which are too clunky or unnatural when translated to english.
