You just need to think of it from a reasonable perspective. How many people in the world? How many do you actually know? You can say they're related (mother son) so her associates should know him BUT do you know the children of your friends or their parents? Let alone just work acquaintances knowing kids/parents unlikely.
He may be powerful but it's not like celebrity fame where he's on the TV or endorses his own makeup brand. It's more infamous where you need to be a member of that world to actually know him. Even then pretty much no one recognizes him by face alone. Almost everyone who knows his face is either dead or his friend. Author also made him have a new look BUT for some reason it's been all but forgotten. His current look isn't even real anymore this was his old body's face. (Chapters 130-140ish but idk why they're not on Mangadex anymore)
