I have a lot of trouble imagining how a manga character could look like a specific real life person, considering how they usually look all the same if you shave their heads, but whatever..

Though that situation would never be possible to happen anyway.
After all the sketch was made upon the agreement that it is for the little sister personally and a private matter.
So it is not the little sister that has to persuade the artist NOT to use the sketch as a model but the artist that must persuade the little sister to be allowed to use that sketch for her work.
Otherwise the artist would risk having her whole work being removed due to disregarding the right for privacy that the little sister has.

There usually is that line at the beginning of every manga and the like saying "This is a work of fiction and all resemblance of any character in the story to any real person is not intended and just a simple coincidence." not without a reason after all.
And this situation would be exactly that kind of situation that that line is meant to explain is not the case.

Exactly spot on. Artists loose their career just for the notion they did a copy of someone else's scene on a single page. They wouldn't risk their livelihood just to use an image like that. Still grateful for the loli tentacles though.

There's also the fact that she's a minor. If the publisher found out she'd used a nude drawing of a minor as a model for a character in lewd situations against that person's wishes? They'd have to cancel the series immediately just on principle. They're extremely averse to that sort of controversy, especially the biggest publishers.
