Kaka is used to address senior military officers. You hear it a lot in Legend of Galactic Heroes or other series with a lot of admirals. It generally gets translated as "Your Excellency," so it's probably also used for high-ranking civil officials who aren't royalty, but they don't turn up as often in anime/manga.
