How he really feels about her ("secrets") turned out to be less-than-flattering ("like being stabbed in the heart").
So he, the wearer, is not the one getting stabbed, but others who hear his true thoughts.

It's symbolic for now, but this manga wouldn't surprise me if it turned literal.
