Her mom is definitely the Queen. You can't just foreshadow her mom being strong and then introduce a mysterious 'queen' villain character without it being connected somehow.

So to get the obvious, for-sure part out of the way . . . so this "queen" injured dad so he never recovered and needs meds, and since then he's been spending all this time raising his girl to be tougher than he ever was and take her down. Whether "queen" is also the mom . . . well, we know she was major tough and isn't on the scene, so that fits. But I dunno, his expression when he alluded to her was kind of too dreamy to be thinking of a monster who nearly killed him, I would have thought.
