This may have given us some decent insight into this dude's character. We learnt that he thinks of himself as pathetic and good for nothing. It's possible that the reason he doesn't respond to Anjo's advances is because he doesn't feel like he's worth someone's affection.

While it feels exaggerated - and generally is, there is the severe tendency towards overworking the Japanese often show. We're talking a country where people have dropped dead from overtime after all.

So, probably, it's less "bit of a fever this morning = drop" and more "ignore the first 2-3 days of feeling pretty bad because you have to work/study" so that even a mild one finally takes you out eventually.
